Who we serve: Adults 60+ and adults with disabilities seeking to live safely at home.

  • Care Navigation: Benefits screening (Medicaid waivers, SNAP), PCP & specialist linkage, durable medical equipment referrals.

  • Home Safety & Accessibility: Fall-risk checks, grab bars referrals, accessibility plans.

  • Support Coordination: Transportation scheduling, prescription pickup coordination, meal resources.

  • Social Connectedness: Friendly calls, caregiver respite referrals, volunteer companions.

Reentry Support (Returning Citizens)

Who we serve: Individuals exiting jail/prison within the last 12–24 months or preparing for release.

  • Documents & Basics: IDs, birth certificates, social security cards, banking.

  • Housing Pathways: Transitional housing referrals, landlord engagement, roommate matching.

  • Workforce: Resume, interview coaching, employer partners, record-friendly hiring.

  • Legal/Records Navigation: Probation/parole compliance, expungement workshops, rights education.

  • Wellbeing: Peer support, mental health referrals, recovery resources, family reunification.

Outcomes we track: Housing stability, ER avoidance, ADL/IADL support goals, caregiver stress reduction.

  • Crisis Navigation: Eviction prevention resources, emergency food/transportation referrals, safety planning.

  • Case Management: Budgeting, benefits access, goal planning,

    life/co-parenting coaching.

  • Childcare & Education Linkages: Early learning, after-school programs, tutoring, IEP/504 navigation.

  • Economic Mobility: Job readiness, short trainings, employer connections, micro-grants.

Who we serve: Single mothers and caregivers facing housing, income, or safety instability.
